Great! Here is a breakdown of how to enroll in services at Sprouts Therapy.

We cannot accept new clients  needing speech therapy only at this time. Our speech therapy waitlist is at capacity. We will update this announcement as soon as openings become available.

2 | Paperwork

After speaking to a member from our Client Support Team, we will send online forms to be completed for your child’s evaluations and/or services. These forms include:

  • Sprouts Intake and Developmental History Paperwork (Required)—sent to your email from an external HIPAA compliant website—IntakeQ.  Please make sure to check every form is completed, as there are multiple forms to start services
  • Physician Referral or Insurance Authorization (Only required if using your insurance for coverage or reimbursement)
  • Insurance card or DoD card (front and back of cards needed) (Only required if using insurance for coverage or reimbursement)
  • Early Intervention or Department of Education Support Plans (if applicable): i.e. IEP, 504 plan, IFSP
  • Existing evaluations (if applicable) 


**Has your child been receiving services at another location/does your child have an evaluation dated within the last 1.5 years? Does your child have an IEP from the school district?  If so, please complete and return the intake forms along with a copy of your child’s evaluation.**

Once we receive all forms and process your new client file, our admin team will reach out to you within 1-2 weeks regarding scheduling your first session and/or evaluation!
