Evaluations

For private pay clients

Your OT Evaluation will include a 2-hour evaluation/parent consultation along with an initial Occupational Therapy report. You will receive the evaluation report in a PDF format, via email, two weeks after the initial evaluation. You will be invoiced at the time of report delivery.
Your Speech/Language Evaluation will include a 1-2-hour evaluation/parent consultation along with an initial Speech Therapy report. The evaluating therapist, based on your child’s presentation and performance, determines the length of the evaluation. You will receive the evaluation report in a PDF format, via email, two weeks after the initial evaluation. You will be invoiced at the time of report delivery.

In-Network Insurance Clients

Your OT Evaluation will include a 1-hour evaluation/parent consultation along with an initial Occupational Therapy report. The cost of the evaluation will be billed to your insurance company. If payment is not received from your insurance company, you will be responsible for payment. The evaluation report will be emailed to you in PDF format two weeks after the initial evaluation.

Sprouts Therapy will conduct your child’s evaluation with professionalism and care. We will utilize one or more of the following standardized tests during your child’s evaluation, depending on their needs and diagnosis.

Occupational Therapy Evaluation Assessments

Sensory Profile 2: Provides you with standardized tools to help evaluate a child's sensory processing patterns in the context of everyday life. (Ages: Birth-14:11)
Peabody Developmental Motor Scales, Second Edition (PDMS-2): Combines in-depth assessment with training or remediation of gross and fine motor skills. (Ages: Birth-5 years)

Beery-Buktenica Developmental Test of Visual-Motor Integration, Sixth Edition, The (BEERY VMI): Helps assess the extent to which individuals can integrate their visual and motor abilities. (Ages: 2:0-99:11)

Bruininks-Oseretsky Test of Motor Proficiency, Second Edition (BOT™-2): Delivers a most precise measure of motor skills, both gross and fine. (Ages: 4:0-21:11)

Additional Occupational Therapy Assessments

The following tests can be administered based on parent request or therapist recommendation at an additional cost due to time required for test administration and scoring.
Sensory Integration and Praxis Tests (SIPT-WPS): Offers the most complete and flexible assessment of sensory integration available (Ages: 4:0-8:11)
Sensorimotor Performance Analysis (SPA)- This criterion-referenced assessment is designed to provide a qualitative record of individual performance on gross and fine motor tasks. (Ages: 5 years- Adult)

Speech Therapy Evaluation Assessments

Preschool Language Scales, Fifth Edition (PLS™-5): An interactive, play-based assessment that provides comprehensive information about language skills for children at high risk for moderate-to-severe language delays. (Ages: Birth-7:11)
The Verbal Behavior Milestones Assessment and Placement Program (VB-MAPP): A criterion-referenced assessment tool, curriculum guide, and skill tracking system that is designed for children with autism, and other individuals who demonstrate language delays.
Kaufman Speech Praxis Test (KSPT): A norm-referenced, diagnostic test assisting in the identification and treatment of childhood apraxia of speech. (Ages 2:0-5:11)
